A Special Incident Report (SIR) documents serious or unexpected events that affect the health or safety of a person receiving services. Service providers, regional centers, and the Department review these reports to identify risks and improve safety.
The Department will host two online training sessions on the updated Special Incident Report (SIR) regulations, which take effect May 1, 2026. The March 24 and April 1 webinars are designed for service providers and will cover:
- Overview of the updated SIR regulations
- Potential impacts for vendors
- New Vendor Guidelines for Reporting Special Incidents
